These forms are for the use of Rotary Clubs in our District and the District itself who to run an event, such as a fundraiser, in partnership with the Rotary D7475 Service Foundation. This enables the event sponsor to take advantage of the 501(c)(3) status of our Foundation.

We've also provided copies of our most recent IRS 990s. Our current name will be shown in the IRS 990 filed for 2021-2022.
